Congratulations on your recognition as a New York State Patient-Centered Medical Home! NYS PCMH Recognition is a significant achievement. Practices that are recognized as patient-centered medical homes demonstrate their commitment to patient-centered, coordinated primary care.
It is important for practices to maintain PCMH operations year round, as NYS PCMH Recognition must be renewed annually. NYC REACH encourages all NYS PCMH Recognized practices to attend a free PCMH Sustainability Workshop to learn best practices for maintaining PCMH operations, how to align PCMH with other programs, and how to maintain NCQA PCMH Recognition through the annual reporting process.

Best practices and tools to sustain PCMH workflows, including from a financial perspective

How to implement additional workflows that support PCMH

How to apply the PCMH foundation to new population health and value-based programs

How to prepare for next NYS PCMH Annual Reporting to NCQA
NYC REACH strongly encourages practices to attend this training at least once a year. NYS PCMH Recognition must be renewed annually. Click here to check your PCMH Recognition status.
